Was your player removed from a game or practice because of a possible head injury? Before a player can return to play a doctor with concussion training must release your player whether you believe your player has a concussion or not. Please have the physician fill out this form and return to the Regional Safety Director or Regional Commissioner before player can return to practice or game.
